Psychological Evaluation for Third-Party Reproduction Candidates

Intended Parent Consultation

As recommended by the American Society of Reproductive Medicine (ASRM), intended parents attend counseling prior to using donor gametes (egg, sperm or embryo) or a gestational carrier. According to Utah law, intended parents with a gestational carrier are required to participate in psychological counseling prior to completing the pre-birth order/assignment of parentage.
This appointment is 1-2 hours.

Gestational Carrier Surrogate Evaluation

As recommended by ASRM and Utah law, gestational carrier candidates participate in a psychological evaluation. This evaluation is an opportunity for psychologists to provide education and informed consent about the surrogacy process and assess psychological preparedness and risk for being a gestational carrier.
This evaluation is a 2-hour interview with the gestational carrier and partner (if married).

Personality Assessment Inventory — This assessment is recommended for any gestational carrier that is not a first-degree relative to the intended parent.

Donor Gametes
Sperm, Egg, Embryo Evaluation

Per ASRM guidelines, it is recommended that all gamete donors participate in psychological counseling prior to donating their gametes in known and unknown arrangements.

Joint Sessions

Following the consultations and evaluations, parties may elect to participate in joint sessions (for intended parents and gestational carrier or gamete donors) to discuss and review expectations, future plans, and any life-changing events during the course of the surrogacy or gamete donor process.

Self-Pay

I do not accept any insurance. All payments are self-pay. I am happy to provide you with a superbill if your insurance will reimburse you for out-of-network coverage.
